What is Linkages?
A Program that provides individualized assessment and plan of care for the frail elderly or disabled adult that is at risk of being institutionalized.
Functioning as a “gap filler”, this program assesses individual needs and connects case management clients to existing community services such as, In-home services, transportation, housing assistance, supplemental food/meals and respite for caregivers.
In addition, Linkages’ may help clients obtain medical equipment, assistance devices or communication devices that help to ensure individual independence.
The Area Agency on Aging also offers free Information & Assistance for seniors, caregivers, veterans and disabled adults. The Senior Information offices are co-located with the Linkages Case Management and Family Caregiver Support programs.
Who is eligible?
Linkages services are not based upon income level. There is no fee for services. Those who can afford to provide a cost share will have the opportunity to do so. No one will be denied service if they cannot contribute.
What can a caller expect from the first call?

If information and referral is the only need, the Area Agency staff is happy to provide the information and referrals for needed support services within Stanislaus County. The statewide toll free senior information number 1-800-510-2020 can connect you to Area Agencies throughout California.

Based upon this initial phone interview, if you qualify and agree to become a Linkages client, you will then be placed onto the existing waiting list to be assigned a case manager. (Can be 3 to 6 months).

We can assist clients on the waiting list with caregiver respite and in-depth information and assistance until they are assigned a case manager.

When assigned to a case manager they will call to arrange for an in-depth interview/home visit to better assess your individual needs/concerns.

Finally, the Linkages Case Manager will develop and review your care plan and with you. Once you agree to the plan, they will implement the planned care/ services that Linkages agrees to provide. The case manager will follow-up with you at least monthly by phone and quarterly in person.
The program also provides for the purchase of supplemental services on a limited basis including:

Minor home modifications or assistive devices that help caregiver accommodate illness or handicap.
Family caregivers are part of a rapidly growing, very large group of people who can support and learn from each other. Using the support groups, information and respite services offered through the Family Caregiver Support Program, caregivers are able to better care for themselves, and by doing so, improve the quality and availability of care for their family members.
