LINKAGES CASE MANAGEMENT

The Linkages Program assists and promotes a better quality of life for persons 18 years or older as well as seniors who may have special needs. These individuals are not eligible for other case management services. Linkages advocates independence and protection of persons who might otherwise require institutionalization.

What is Linkages?

Functioning as a "gap filler", this program assesses individual needs and connects case management clients to existing community services such as, transportation, housing assistance, meals and daycare. In addition, Linkages’ focus is to obtain medical equipment, assistance devices and communication devices that help to ensure individual independence.

Linkages also serves as an information referral service for Stanislaus County, providing information, support services and referral to agencies to meet your needs.

Linkages services are not based upon income level. Those who can afford to provide a cost share will have the opportunity to do so. No one will be denied service if they cannot contribute a cost share.

What can a caller expect from the first call?

If information and referral is the only need, Linkages is happy to provide the information for the most commonly needed support services and agencies within Stanislaus County.

If you are calling about yourself or someone you care about, you will discuss with a trained and experienced professional the purpose for your inquiry.

Based upon this initial discussion, a follow-up meeting will be scheduled to specifically address the situation and needs.

The final result is a Linkages Case Manager will develop an individual care plan and will implement all aspects of the care plan.
