Provider Program:

FAMILY CAREGIVER SUPPORT PROGRAM

If you are caring for a loved one age 60 or older, you are eligible for the many services offered free-of-charge by the Family Caregiver Support Program including:

Specialized advice, information and referrals to available services in your community
Family consultation and care planning
Caregiver education and training
Support groups
Short term respite for caregivers
Annual caregiver retreat with respite services

The program also provides for the purchase of supplemental services on a limited basis including:

Respite time off:  Respite care including adult day care, overnight stays in a facility, respite camps/weekend retreats, and other respite options.

Transportation of the caregiver for respite
Minor home modifications to help caregiver accommodate illness or handicap
Counseling
Placement assistance

Family caregivers are part of a rapidly growing, very large group of people who can support and learn from each other. Using the support groups, information and respite services offered through the Family Caregiver Support Program, caregivers are able to better care for themselves, and by doing so, improve the quality and availability of care for their family members.

Information and Assistance:

We will provide information and assistance on all aspects of care.

Family Consultations:

We assist caregivers in exploring immediate and long term care options, and provide support and guidance in decision making.

Support Groups:

In addition to facilitating our own support groups, we provide referrals to other existing support groups and assistance in establishing groups where needed.

Legal & Financial Consultations:

A free consultation is available with an attorney experienced in issues relevant to caring for a brain-impaired adult. The attorney will provide advice in such areas as establishing durable powers of attorney, conservatorship, financial planning and clarification of community property laws.

Family Counseling:

Short-term counseling with family members is available to help identify problems and explore solutions. Assistance is given in obtaining on-going counseling when needed.

Respite For Caregivers:

Subsidies available for in-home care, day care and transportation to assist families caring for a brain-impaired adult at home.

Training & Conferences for Families & Professionals:

Specialized education about research, diagnosis, behavioral management, stress reduction, and other issues related to caregiving can be arranged.
